Case Management Services
ICM Associates, Inc. pioneered the Case Management Model of assisting individuals navigate the next step of their recovery process nearly a decade ago.
At ICM, we do not do "recovery coaching" or "sober coaching" and our services are not limited simply to providing our clients with local transportation or referrals to specialists. We are a true case management firm which guides each individual client, with the assistance of their professional treatment through the early phases of their recovery.
Our Approach...
When a person is being discharged from the secure enviorment of a treatment facility back home or into another recovery-based community, the transition from one very structured phase into the next can be extremely stressful.
Each new client is assigned a primary case manager with whom they meet privatly at at times, in session with their treatment providers. We work closely with the family and support system as well and perform a "case management needs assessment.
Each client has unique needs upon transition, and ICM Associates works with them to identify a plan of action that is concrete, relevent, and specific to their needs. If needed, ICM will create a treatment team based on the clients needs which may include:
* Mental health professionals such as psychiatrists, therapists, and outpatient programs.
*Other professionals such as nutritionists, acupuncture physicians, personal trainers, and other medical specialists.
*Sober living residences and outpatient programs.
*Misc. services such as transportation assistance, educational/vocational agencies, employment services, and other community programs that are available.
Together with the referent, the client, and their support system, a plan for ongoing recovery is carefully mapped out. The ICM case manager meets individually with each client in a neutral setting to enhance their current lifeskills base and to develop other skills deemed critical to ongoing stability.
Create, Coordinate, Manage...
Once the aftercare plan and continuing care plan is developed, ICM case managers focus on coordinating the overall plan and ensuring that it's feasable and without barriers to the client.
In addition to working individually with our clients, we also serve as the designated "point of contact" for communication among providers. We ensure that all providers are kept updated on their client's care at all times.
Our clients benefit from this maximized and enhanced communication as they can be assured that all aspects of their recovery is being carefully paid attention to, allowing them to concentrate on staying well.
